น่าจะเป็น Ichthyosis hystrix เห็นว่า spare ที่หน้า ฝ่ามือ ฝ่าเท้าด้วย เลยคิดว่าน่าจะเป็น Ichthyosis hystrix, Lambert type นะครับ
Ichthyosis hystrix, Lambert type
Also known as Ichthyosis hystrix gravior or porcupine man. This disease is characterised by spiny scales which cover the entire body except the face, genitals, palms and soles. The only known cases were in Edward Lambert (known as the porcupine man) who was exhibited in front of the Royal Society in London in 1731 and three generations of his descendants. No cases of this disease are now known though some experts believe that it may have been a type of epidermolytic hyperkeratosis. Ichthyosis hystrix A 2½-year-old male child presented with generalized scaly eruption since three months of age, which were progressively increasing. These were linear as well as circumscribed dark brown to dirty looking hyperkeratotic and verrucous plaque lesions distributed bilaterally and symmetrically, over the neck, trunk and extremities (Fig. 1). Some of the individual papular lesions were spiny. The linear arrangement of lesions was conspicuous over the limbs especially over the dorsa of hands. The scalp, face, palms and soles were spared but genitalia was involved. The hair and nails were normal. Skin biopsy showed features of epidermolytic hyperkeratosis.
